On the above date, the above named officer responded to the above location regarding the above investigation. Upon arrival first responders Medical units were already on scene and advised that Ms. Martinez was unresponsive to their attempts to revieve her. Further investigation showed that Ms. Martinez had an extensive medical history and had been sick for several months. Investigation with the office of the medical investigator did not show any signs of foul play. For further information refer to officer's report.
